If a separate sports physical is needed: ✔️ $45 (not billed to insurance) ✔️ Payment due at the time of service ✔️ Immunizations are not included 📋 Don’t forget to bring your school sports form!

  • Sports season will be here before we know it, and appointment times fill up quickly!
