Sports Physicals Made Easy!
If your child is due for their annual Well Child Exam, we may be able to complete their sports physical at the same visit!
If a separate sports physical is needed:
✔️ $45 (not billed to insurance)
✔️ Payment due at the time of service
✔️ Immunizations are not included
📋 Don’t forget to bring your school sports form!
